I'm sharing my experience of 3 weeks ago changing over my UK license to Bahrain.  Hoping it will help someone.

You need to go to the Isa Town driving School DRIVING SCHOOL BAHRAIN (TRAFFIC&LICENSING/DRIVING LICENSES-A'ALI)

Copy of your license from your country back and front. If not there is a photocopier there that takes 100fils per copy
26.5 BHD cost for test and license

First thing, don't go on a Sunday, too many people apparently a lot of Saudi people get it done then.
Next, get there at 7am or even 6.45am.  Opening hours are 7am to 2pm. I got there at 7.55am one Sunday and it was full
3. On your left, after entering, will be a counter where people are lining up. There tell the gentleman what you want to do and he will give you the firm and a ticket to queue for the eye test.
4. This queue moved quickly.  Have your form filled in and the ticket ready.  You must have a CPR to apply for a Bahraini licence.
5. When it's your turn you go into a dark room and you read letters from a wall.  Hopefully all will be well.
6. Go and queue again and this time it's to get the licence.
7. When your number comes up, go to desk and  form, fee, CPR, license and copy will be taken and you new license printed. Valid for 5 years.
